Objectives

Smart Monitoring

Development of new sensors and technologies to monitor patients, frail populations and environments (i.e. home, Point of Care, PoC, mobile units) that provide continuous real-time data collection and integration.

Prevention and Diagnosis

Development of new technologies and AI-methodologies aimed at improving early diagnosis, implementing digital pathology and allowing their scalability at pathology and/or territorial level.

Personalised Medical Treatments

Development of advanced treatments for orphan cancers, chronic conditions dedicated to specific target populations and territories.

Technological Enhancement and Transferability

Development of methodologies for heterogeneous health data management, protection, interoperability and for data integration with existing or developing information technology platforms.

The Spokes

The project activities are organised in four Spokes dedicated to the implementation and development of innovative technologies targeted at specific diseases and territorial communities that will have a direct impact on: a) the strengthening the interoperability of health data, b) the development of digital tools and diagnostic and monitoring devices adaptable to different territorial contexts and reference populations; c) the development of innovative therapeutic strategies for specific chronic diseases in target populations.

Fragile and chronic represent a large number of patients who need continuous care from health systems and healthcare providpopulations ers.

Italy is the second Country worldwide for longevity, with an associated increase of chronic non-communicable diseases (NCD). The World Health Organization (WHO) estimated that NCDs such as cancers, degenerative diseases, cardiovascular and lung diseases are responsible for 71% of all deaths worldwide.
The National Chronic Disease Plan (NPC) indicates that the more frequent pathologies or comorbidities in the chronic and fragile population are represented by cardiovascular diseases as a whole (32.8%), chronic respiratory diseases (24.5%), diabetes (20.3%) and cancers (12.7%).
Fragile and chronic patients are a challenge for the health system in terms of organisation, process efficiency, quality and effectiveness of care, and health economics.
The efficiency of the medical care at national level shows peaks of excellence and disparities due to the country’s geographical structure, variations in population density, connectivity services, lack of qualified personnel and local socio-economic conditions.

The Anthem Initiative is part of this context with the aim of filling current gaps in the health care of fragile and chronic patients within specific territories and communities with high incidence of orphan diseases.
